Authorities have confirmed the identity of the man who died following a multi-vehicle crash in Oak Park. Thomas Cremeens, 48, was pronounced dead at the scene after the incident on Friday morning.

Officials reported that the crash occurred just before 9 a.m. at the intersection of Harlem Avenue and Lake Street. Investigators said Cremeens was driving a Toyota 4Runner at a high speed when he swerved to avoid a pedestrian, striking a car and a Pace bus in the northbound lanes.

Emergency responders and bystanders discovered Cremeens inside the vehicle with a fatal gunshot wound. A gun was found on the floorboard, and he was pronounced dead at 9:13 a.m. Fourteen other people sustained injuries, with eleven transported to local hospitals and three treated at the scene; none were considered life-threatening.

Oak Park, River Forest, and Forest Park police closed nearby roads for several hours to manage the scene. The circumstances surrounding the gunshot remain under investigation.
